Student Parking Form

All Grades:

Students need to submit their photo ID, vehicle registration, and proof of insurance. These documents must be in JPEG, PNG or PDF format. 

South CTA Sports Participation Form

1. Students who participate on a CCSD sports team must provide their own transportation to and from that school.
2. Students must complete an Athletic Packet from their zoned school and our South CTA Sports Participation Form for each season (fall, winter, spring) that they will be playing a sport. South CTA does not require an Athletic Packet to be submitted.
3. Only NIAA approved athletes will be allowed to be released early for travel time to games.
4. Students are not released early during intramurals, for practice sessions, or for pep assemblies.
5. Prospective student athletes who wish to play college sports as freshmen in Division I and II schools must consult their zoned school coach for eligibility requirements.
6. South CTA does offer academic courses for high school credit that will qualify students for NCAA consideration.
7. NIAA rules and regulations determine a student's eligibility.

Current, actively enrolled South CTA students can conveniently request their official high school transcripts directly through our school’s Registration Office by submitting our Transcript Request form.

However, if you are no longer a student at South CTA, all transcript requests must be processed through the district portal at transcripts.ccsd.net.
