If your student has one or more of the following, please contact our Health Office so that the proper forms may be filled out and documented for future reference:

  • A medical condition (New or not previously disclosed)
  • A prescribed medication that must be administered by an adult
  • A prescribed medication that may be self-administered
  • An over-the-counter medication that may be self-administered
  • An injury which would require the use of any equipment/accessory to increase quality of life, such as:
    • Crutches
    • Wheelchair
    • Oxygen Tank
    • Scooter
    • Tinted lenses (light sensitivity), etc.
